I've been looking into capturing some old Q2 demos with KMQuake2 and quakecapture patched in. The problem I'm facing is that if I want to record any decently long HD captures (20+ minutes, 720p), it gets stuck at a 4GB avi file.

Anyone else run into this problem and have a fix?

4GB? it might be filesystem issue. FAT32 partitions allow maximum filesize that big. switch to NTFS partition or make it autospit video into multiple files. otherwise i dont know KMQuake2 and quakecapture software and hoping others could help you.
