Unless you desperately need one more verifier, I'm going to recuse myself from this. I've taken a good look at the video, and I simply don't feel qualified to judge the level of play here - it's like a different game than what I play. =)
As far as the basics: video quality good. Level of play skill and route optimization, from what I can tell, is /ridiculously/ good.
We have no easily accessible run for the game on the site currently (one leads to an outside site that's a dead link, and the other is a zip file - a zip file! My god, it's the Dark Ages), and it's faster than the zipped record, so for what my opinion's worth, it seems like, to borrow a phrase, a 'no-brainer'. Accept.

As you would expect from an improvement in a game this popular, this run is quite impressive. It uses all the shortcuts and tricks I knew about and many I didn't. Every aspect of this run is the definition of precise. Other than the most minor errors, this is flawless.

I found 2 weird occurences in this run. At 3:31 on the video, he picks up an Adrenaline, which seems to do its job of increasing his max life by 1 and giving full life. Later, at 4:20, he picks up enough health to max him out at 101, but it instead only brings him to 100. Later adrenalines properly increase his life. I wouldn't say it's a sign of cheating, as it's his loss of 1 hp for the whole run, but it's weird nonetheless.

@health problem: when doing this run I didn't use auto-saves between levels, instead I used a script to get the right stats for the next level because of various things. It seems I overlooked a slight mistake in the script and didn't notice it until later. I have no way of fixing this now. Like he mentioned it is a disadvantage for me as I have 1hp less on one of the maps.
@msg of me getting killed: Like I mentioned before, because of my technical problems some of the levels through which I have to pass twice or more I had to record the levels with fraps the second way around. And it seems that it takes some time for quake to clear everything from the screen from the previous attempt.
