I too have been playing the arcade VC version of Ghosts 'n Goblins on the Wii lately. For fun I tried a run on the hardest difficulty setting (Very Hard). Ugh, I had huge issues getting past the first level, which was not a problem on Normal. The increased number of monsters and the speed increase of them and the projectiles really screwed me up badly. Good thing I'm not spending real money on credits like I did back in the day.

Other games worth a mention for me include God Hand on hard mode (first loop), and Alien Soldier.
I would say Blast Corps as the platinum medal challenge is about the only gaming achievement that I have tried and still eludes me.

I'm just putting it out there but Battletoads is actually deceptively easy. If you play it enough times and memorise all of it and know what to do and when to do it then by the time you finish it it isn't actually that hard. I say it's deceptively easy as it completely depends on your skill and memory as opposed to say unfair and cheating AI.
The hardest game I've ever played would be I Wanna Be The Guy, but I assume that goes against the point of the thread Tongue

If we're talking commercial game, that'd have to be Super Ghosts N' Ghouls. I couldn't get past that water part 5 minutes into the first level >.<
No longer a threat to society
This is going to sound weird, and I'm probably the only one who hasn't finished this game, but... Resident Evil.

It's not hard in terms of difficulty, but I couldn't figure out the puzzles and the atmosphere just seemed to take my focus away from the details in the room and the solutions for the puzzles, and put it on watching my back and constantly checking my ammo. It's the only game that to this day, freaks me out and I can't play it. I don't know what it is. I've even done other resident evil games no problem. Just, that first cut scene with the zombie turning around does me in. Oh, and I'm a 200 pound multi discipline black belt.

Prinny can I really be the Hero or Super Meat Boy for games I've actually beaten. I beat a few bosses  on hells finest for Prinny and beat the majority of super meat boy with a keyboard. 

The hardest game I haven't beaten is probably metal slug 3 on xbox that may change later this year though

Edit: Actually on second thought Sonic the Hedgehog 2006 is probably the hardest game I've ever played

Yeah that's one of the hardest games I've beaten. The story mode is unbelievably hard on normal difficulty and practically impossible on very hard. That said I didn't actually have that much trouble beating all the cups on the hardest difficulty. The game became a lot less intimidating as time went on because I found my optimal racer setup, got familiar with the tracks and was generally just better.

Any roguelike basically, but most of all UnReal World. The game is so biased against the player it's ridiculous. Cheesy Kinda realistic though!

Other than that, Mushihimesama Futari Ultra mode. Can barely reach stage 2 midboss so far; I'd say the most I could ever hope for would be stage 3 midboss. God mode is much easier (and hella fun, too!).

Touhou: Imperishable night on easy mode and Crash Bash have now joined the list of hardest games I've ever beaten.  Now another 120 or so percent to go on Crash Bash...

QWOP! Just kidding. The hardest game I ever played at the time that I played it was Pokemon Yellow Version. I didn't know how to catch Pokemon and to this day am not sure how I beat the first gym, let alone the game.

Definitly CHAKAN: THE FOREVER MAN........i thought about a speedrun but beating this game normally is a pain .  I wrote an email to Mike because i want to see a run of this game. But it is so buggy and freakin` hard that
no one seems to have the nerves to do this. First half of the game is quite doable, but then it becomes a nightmare.  And oh this crappy ending after u beat it finally =)

Spider: The Video Game, for PS1. This is probably the hardest one I have ever completed. It was never the type of difficulty where I was always thinking to myself "Why did they make this so unnecessarily difficult?" Instead of luck or clunky controls to blame for the difficulty(which isn't a part of this game thankfully),  aka "fake difficulty", they put 0 checkpoints inside the levels. This forces you to memorize things and the game will always punish you for trying to do too much too fast. I like this kind of difficulty. If you know what you are doing you can do something the first time, then its just getting over the absurd reaction time this game expects you to have, but that's just skill. If you are into PS1 titles this one is a rare one you should pursue if you are looking for a challenge.

Super Ghouls 'N Ghosts.  2 hits and you die.  Hard jumping mechanics.  However it is still a really fun game.

I wanna be the Boshy or Touhou: Mountain of Faith... Can't decide which one. Both require a ton of memorization, execution and strategy.
The hardest game i´ve beat is probably IWBTG on Very Hard mode. Dark Souls was also really damn hard.

But Jak II is right now the hardest game I've ever played for any length of time (and I've beaten it too)
