Very Easy, Easy, Normal, Hard, Extreme, European Extreme.  Probably could have looked at gamefaqs or something =\

I love YaBB 1G - SP1!
As the difficulty increases:

- Better overall A.I, enemies will be able to hear and see you from further away.
- The harder the difficulty, the less stuff you'll be able to carry around, i.e less bullets, grenades..etc
- More guards in pretty much every area
- It'll take more bullets to kill guards and tranquilizer bullets will take more time to knock them down, unless you shot 'em in the head.
- Bosses will be harder to beat, i.e it'll take more bullets to stamina kill 'em on the harder difficulties, more ocelot unit soldiers..etc
- Less overall health
- IIRC you'll even lose stamina faster compared to the easier difficulties.
- Of course certain ranks are only achievable on the harder difficulties, i.e you can only get Foxhound when using Extreme difficulty. 

That's all I can think of for now, it's been a while since I played the game so I might have missed something.
Why does this keep happening to me?
Another thing is that the tranquilizers have less of an effect when they work. I.e on E Extreme, a sleeping guard having being shot in the head with a tranq will only remain asleep for about 2-3 minutes, maybe not even that. But tranquilizing a guard on Very Easy will put him down for at least bloody twenty. the only way I can wake a guard up on Very Easy is to coolant spray him till he wakes up.

I think you're referencing Metal Gear Solid 2 there. There is no coolant spray in MGS3.

But the biggest difference I find in the higher difficulties are the number of guards. Outside Rassvet (factory where Sokolov is held) on very easy, you'll be lucky to bump into two or three guards. On Extreme though, I'm sure there's about 5-7 around the place. It's quite difficult to account for them all.

You definitely have less health to start off with on extreme. Obviously if you take a serious injury and heal yourself surgically, your maximum health increases very slightly, at least in my experience it does.
Less ammo, grenades etc are also a very noticeable thing.

I'm not sure about if your stamina drops faster, but I think it does, could be corrected on this however.

Guards are far more intelligent. On very easy you'd need to wave in their faces to get them to see you, but Extreme brings in a more lifelike feel, and they can spot you easier. KGeeB is right about tranquillizer effects though, but honestly, why would you want to wake him up? xD

Also, I believe you are more prone to serious injuries. Like falling off a tree on very easy would have a lot less chance of giving you a fracture or a lasceration, as opposed to extreme, which would be much higher. This I think goes for bullet wounds and things of that nature. Thats something to be avoided if you're going for the high ratings which demand less than 20 or so serious injuries.

I do believe their bullets so slightly more as well. Mind you, you could pass that off to having less health, so the effects are more noticeable. Guards can hear you too as metallicaband pointed out as well. I hope these helped. I can't help but think I've just quoted exactly what the person before me said...

You can prop them up against a wall and punch them in the head for a similar effect.
