Does it make any difference in a map?

I'm in the process of making a map, I've got the normal Player Start, but I'm adding the Co-ops aswell, any reason for the Player Start 2?

No, ignore info_player_start2. According to the code, it gets picked if 'serverflags' is set. Use coop.

info_player_start2 is only used for "returning" to the start level after finishing an episode, i.e. gets you put in the room with the episode enterances rather than the skill halls.
