The only death defying moment is around 15:35 in segment 3 in the DMC3 run where my general sloppiness has me down to a centimeter of my life bar and I still need to dispatch two arachnoids, but it's nothing all THAT impressive.  I did get lucky though with a delayed jump I had in there...
I think that Damien 'Dragondarch' Moody's 1% Metroid Fusion run is amazing. There has to be a few parts of that run worth getting a place in the movie? For example beating Nightmare with no energy tanks?

Is there anything from Contra: Shattered Soldier that was particularly death-defying?  I have always regarded that run with high esteem, and I'm probably going to have to watch it again, but I'd wager heavily that there are moments in it similar to Metal Slug X's "nerves of steel."

Here are some clips I thought might be eligible.  I believe they are worth adequate consideration, if nothing else.

-Sonic Advance SS at about 20:00 minutes even. 

-Turok, segment two, at about 7:50 minutes.

-The original Doom, Episode 3 Mission 4, at about 20 seconds.

That's all I've gathered, for now.  Of course, someone really should wade through the Diablo runs and find some moments.  (Note: At the present time, InsipidMuckyWater is desperately trying to forfeit this responsibility.)

Marshmallow is correct; I made a mistake.  I thought, at that moment, that you were down to two hp, but actually, you are just running out of armor (I think).

I apologize for reviving this topic, but I am going to begin more serious work on this video in the near future.

And I mean that I intend to get this video done before Christmas at the absolute worst.

I've only heard one music suggestion (and I've yet to hear the song suggested), though, so I'd like to find out some more viable music for a video like this.  If you have music to suggest, say it, so I can get some ideas for what to use

If I ever do another frag video I'd probably use the song "Bim Bam Smash" from The Bourne Supremacy soundtrack.  It's an excellent full-of-action song rarely found in movies with a really strong ending and a good length I think (5 minutes).  You can also grab the song for $1 from iTunes and de-DRM-ify it with jHymn.

From what I've watched I don't think anyone else has used it before.  Since I don't know when I'd ever do another frag video feel free to try it out.  I mainly just want to hear it used in a cool video.

Of course there's also a lot of good game music/game music remixes to use, but to me that would be sort of weird to have music from one game playing over a bunch of different other games.

I'm pretty fond of the Parrot-Chute Panic strategy in my Donkey Kong Country 2 run.  The bit between 1:10:46 and 1:10:56 is cool.  Might count as a death-defying moment.

Psh!  My Parrot-Chute Panic owns yours (35:16-35:45)  Although you had one Kong throughout most of the sections by abusing the DK barrel positions so you could fall through the bees, I had NOTHING to work with!  I had to save both Kongs for the final fall to the flag.  I do like my Web Woods right after I get turned into Squitter (37:31-37:41) and the other squitter moment in 20:37-21:22.

I remember there being a better moment anyways in the DKC2 SNES run than the Parrot-Chute Panic.  I'm gonna view it again, but I think it was in Animal Antics (Granted that entire level, the entire Lost World was evil in its own right, major props for that).

Edit:  Found it.  In the Squawks section of Animal Antics (116:32-116:50 according to my Winamp player) you crash through a dragonfly and lose the first kong and when you crash into the DK barrel, the game seems to bug on you and you don't get the Kong back.  I am not sure if you noticed when playing through but at the last stretch you almost get blown into a bee trying to kill it, and the other one you just BARELY miss zooming by.  I don't even think you knew you only had a single Kong, especially when it's the very last level in the run and definitely one of the hardest ones in the game.

Doom 3 run, I can't remember at what point, but Apathy gets bogged down and he is left with exactly 2 health, and he somehow survives killing a bunch of monsters. Excellent skill.

stx-Vile's nightmare episode run of The Shores of Hell. When he pulls off the rocket boost to the door. I spent hours trying to land that once, and he took a risk and did it that far into a great run.

I would also have to say Psychochild's Parrot-Chute panic (sorry SnapDragon, even though your run is.... O_O). I had to rewind it a couple of times to believe it. That far into a run too.... eesh.

Parrot-Chute Panic was never the reason I would have to scrap a run.

It's was always that damn Rickety Race!  That entire level is a damn death defying moment!  Curse you shoddy hit detection!  Curse you to Hell!

In the ninth segment of the LOD Sorceress run, at 2:22 Mephisto knocks him pretty close to death.  Part six at 3:00 minutes contains an even worse moment, but not against a boss.  Granted, the segments are short, and in both clips he has a potion already in the process of restoring his life, but still.

Also, in the infamous Mario Bros. 1 (5:06 or 5:09) run done by Sequin, there are particularly miraculous moments at 3:26, and at 3:54, even though it's obvious that he, somehow, knew what he was doing. Finally, at 4:58, the impervious Trevor dodges not one, but two fireball walls that at least APPEAR to make contact with Mario.  Actually, there are other, similar moments through the duration of the run, but I favor these three.

I have a few moments in that SMB1 video that just made me go nuts.

At 0:53 into the video, simply watch those two jumps....

At 3:10 in the video, watch it until 3:13.... @_@;

At 3:28, watch those jumps and that duck.... omfg, the duck until about 3:29. Fucking amazing.
I like to nominate Trihex's latest Yoshi Island run, namely at 53:24 to 53:30 in world 4-3 where Yoshi's still recovering from a lost Baby Mario, he's jumping around and he gets hit by a spit out Spiny.  He then avoids falling to his death and BARELY makes it back in time to save Baby Mario.  He was literally sitting at 0 seconds on the dang clock!  If that is not cheating death (TWICE) then I don't know what is.

While this technically doesn't count as death defying, it may qualify as ruined-run defying.

I finally got around to watching the Banjo-Kazooie run, and while I haven't finished viewing it, I think it's appropriate to nominate the run for its moment beginning at about 57:20.  (BTW, Marsh, I don't know how you got the guts to do this run, but I'm glad you did.)  What's-his-face came about as damn close to a draw as possible; I assume that would have mandated a restart.

Obviously, such an event would have quite surely ruined the demo, so there ya' go.

I had multiple close calls in my first game of Chrono Trigger, but it was on emu and wasn't a speed run. It was still extremely close.

I HATE VILE'S MINIGAME. Oh my god...it cost me at least two practice runs. Maybe I just suck at it, but it seems to be a huge amount of luck involved. Not to mention, a tie = a loss.
