As for 31-44, maybe Tri-hex's goofup in Yoshi's Island? I can't remember where it happened, but Tri-hex basically got cocky and tried to pull off some move and died at a boss when he otherwise would have easily been in the clear.

2-8, after he beat the ghost in a bottle. He tried to pile drive so that he'd either be just barely off screen or about to fall off when the game would auto halt his progress (as the boss explodes) but started too soon.
The ghost is trapped inside a pot, FYI.

I agree overall it's a great mess-up, but man ... I really wish that it wasn't the only thing people remembered from my contribution to the 2010 Marathon. But, whatever. I'll take the hit for the sake of a better promo video.
